Eligibility Requirements:
Must enroll between January 12, 2001 and. January 31, 2002.
Must request installation of all new service no later than February 28, 2002 to occur no later than May 31, 2002.
Must commit to a 2-, 3-, 4- or 5-year term of service for new service.
